HIV/AIDS Care and Support Programs

AIDS Emergency Fund:
  The Fund is a safety net to provide emergency financial assistance for basic necessities including utilities, telephone, housing and medications. These funds are accessed when clients do not qualify for State or Federal assistance, or when other funding has been exhausted. The Fund is managed at no charge by The Health Trust is supported by the Walk and UTM.
AIDS Legal Services:
  AIDS Legal Services (ALS) provides free legal representation to low income individuals living with HIV/AIDS in Santa Clara County.  ALS is the only legal assistance program in Santa Clara County specializing in HIV/AIDS-related law, providing assistance to over 300 clients each year in a wide range of legal practice areas:  confidentiality, consumer protection, disability rights, discrimination, employment, estate planning, housing, and public/private health and income benefits.  ALS also conducts community outreach and education presentations and engages in system reform efforts.

The Health Trust AIDS Services:
  The Health Trust is the premier AIDS case management program of Santa Clara County and serves over 50 percent of the individuals diagnosed with AIDS. These services include: planning and arranging for home care in lieu of hospitalization, home delivered meals, monthly food and nutritional supplements through the Food Basket, emergency financial assistance, transportation and housing. The Neil A Christie Living Center is a drop in center for people with HIV/AIDS that provides social support, referrals and trainings designed to help with education and prevention as well as how to survive the diagnosis and ensuing complications of living with HIV.  THT staff includes nurses, social workers, benefits counselors, and housing aides who can make home visits, if necessary, to help clients access all available services and resources. Community Health Partnership:
  The mission of the CHP is to expand access to health services for the medically underserved, encourage prevention and health promotion through service, education and advocacy. As a consortium of community health centers, the Partnership is committed to developing affordable health services that meet the needs of our diverse multi-cultural community.  The HIV Services team is dedicated to providing the community with direct client services such as support groups for transgender people of color, case management services, outreach, and HIV testing; capacity building assistance for partnering agencies; and policy advocacy.  HIV Service programs at the Partnership include San Jose AIDS Education & Training Center, the TransPowerment Program, Alternative HIV Testing Sites, Minority AIDS Initiative, and Santa Clara County HIV Planning Council Support Program. Bill Wilson Center:
  Provides prevention as well as early intervention referral services to high-risk street youth, including gay and bisexual youth, sexually active youth, and youth who have been involved with drugs and/or survival sex. Additionally, through its Centre for Living with Dying program, Bill Wilson Center provides quarterly AIDS retreats and both individual and group counseling and support. Visit website>

HIV/AIDS Prevention/ Education Programs

Asian Americans for Community Involvement (AACI):
 

is the largest nonprofit health and human service provider focused on the well being of Asian Americans throughout Santa Clara County. AACI is the sole agency within the county providing HIV/AIDS prevention services that target the Asian American community, and its goal is to reduce the incidence of HIV infection within this population.  The program also serves other at risk communities of color, substance abusers, and formerly incarcerated populations.  Program components include community outreach; education sessions; HIV rapid testing; individual/online counseling; support/discussion groups; and community mobilization and advocacy. ProLatino:
  is a nonprofit organization that provides Prevention, Education and Advocacy to the Gay, Lesbian, Bisexual, and Transgender (GLBT) Spanish speaking Latino community in Santa Clara County. ProLatino additionally provides support groups on HIV/AIDS and STD's. We also provide referrals to medical, legal, housing, individual counseling and food assistance programs. Visit website>
Billy DeFrank LGBT Community Center:
  The AIDS programs at BDF focus on preventing MN infection In the LGBT communities of Silicon Valley with a particular emphasis on young men who sex with men. The Center’s Internet based intervention program is one of the most innovative prevention programs in nation. Visit website>
ACSV Advocacy Project:
  As the only AIDS-specific advocacy project in the county the ACSV Advocacy Project provides ongoing analysis, education and advocacy on issues of importance to the AIDS service agencies and people with AIDS of Silicon Valley.
